Definition:
Gynecomastia refers to the enlargement of male breast tissue.
It can manifest as a button-sized growth underneath the nipple. The lump may be palpable and tender.
While it can occur at any age, it is most commonly observed during the newborn period, puberty, and older adulthood.
Causes:
1. **Hormonal Imbalance**:
The primary cause of gynecomastia is an imbalance between estrogen and testosterone hormones.
Normally, men produce small amounts of estrogen, which controls breast growth.
If there's an excess of estrogen or low testosterone (hypogonadism), male breasts may enlarge.
2. **Puberty**:
During adolescence, more than half of teenage boys experience some degree of breast enlargement due to fluctuating hormones.
This condition usually resolves as hormone levels stabilize over several months to two years.
3. **Adulthood**:
Enlarged breasts are more common in men over 50. As men age, their bodies produce less testosterone,
and increased body fat can stimulate estrogen production and breast tissue growth.
4. **Other Conditions**:
Certain medical conditions, such as adrenal tumors, alcoholism, kidney disease, and Klinefelter syndrome (an inherited condition), can also lead to gynecomastia⊃2;⊃3;⁴⁵.
How Hormones Affect Gynecomastia:
- **Estrogen**: Estrogen stimulates breast tissue proliferation.
- **Androgens (Testosterone)**: Androgens inhibit breast tissue proliferation.
- An altered ratio of estrogens to androgens, mediated by changes in estrogen and androgen actions, contributes to gynecomastia.
