As a fundamental diagnostic tool in medicine, the electrocardiogram (ECG) is a non-invasive test that measures the electrical activity of the heart. In this overview, we will cover the basics of ECG trace reading and electrode placement, providing a comprehensive introduction for medical students.

ECG Basics

An ECG tracing is a graphical representation of the heart's electrical activity, recorded from the body surface. The tracing is composed of several key components:

  1. P wave: Represents atrial depolarization (contraction of the atria)
  2. QRS complex: Represents ventricular depolarization (contraction of the ventricles)
  3. T wave: Represents ventricular repolarization (recovery of the ventricles)
  4. PR interval: Measures the time between the onset of the P wave and the start of the QRS complex
  5. QT interval: Measures the time between the start of the QRS complex and the end of the T wave

ECG Electrode Placement

To record an ECG, electrodes are placed on the body surface to capture the heart's electrical activity. The standard 12-lead ECG uses a combination of limb and chest electrodes:

  1. Limb electrodes:
    • Right arm (RA)
    • Left arm (LA)
    • Right leg (RL)
    • Left leg (LL)
  2. Chest electrodes:
    • V1: 4th intercostal space, right sternal border
    • V2: 4th intercostal space, left sternal border
    • V3: Midway between V2 and V4
    • V4: 5th intercostal space, midclavicular line
    • V5: Anterior axillary line, level with V4
    • V6: Midaxillary line, level with V4

Lead Placement and Orientation

The ECG leads are divided into two categories:

  1. Bipolar leads: Record the difference in electrical potential between two electrodes
    • Lead I: LA - RA
    • Lead II: LL - RA
    • Lead III: LL - LA
  2. Unipolar leads: Record the electrical potential at a single electrode, referenced to a combination of other electrodes
    • aVR: Right arm, referenced to the average of the other limb electrodes
    • aVL: Left arm, referenced to the average of the other limb electrodes
    • aVF: Left leg, referenced to the average of the other limb electrodes

Chest Lead Placement

The chest leads (V1-V6) are placed in a specific pattern to capture the heart's electrical activity from different angles:

  1. V1 and V2: Record the septal region of the heart
  2. V3 and V4: Record the anterior wall of the heart
  3. V5 and V6: Record the lateral wall of the heart

ECG Trace Reading

When reading an ECG tracing, consider the following steps:

  1. Rate: Calculate the heart rate (beats per minute)
  2. Rhythm: Determine the rhythm of the heart (e.g., sinus, atrial fibrillation)
  3. Axis: Calculate the electrical axis of the heart (normal, left, or right deviation)
  4. Intervals: Measure the PR, QT, and QRS intervals
  5. Morphology: Examine the shape and size of the P, QRS, and T waves

Clinical Correlations

ECG interpretation is essential for diagnosing and managing various cardiac conditions, including:

 

  • Arrhythmias (e.g., atrial fibrillation, ventricular tachycardia)
  • Conduction disorders (e.g., bundle branch blocks)
  • Myocardial infarction (heart attack)
  • Cardiomyopathies (e.g., hypertrophic cardiomyopathy)
